Madame Hydra was in charge of HYDRA. She somehow knew about Wolverine and the Weapon X project. She arranged for Dr. Deborah Risman to create X-23 but the clone escaped.
X-23 returned with Wolverine but it was a trap. She apparently died along with Gauntlet.
